Encompass Health Rehabilitation Hospital of Jacksonville

I cannot say enough wonderful things about Encompass Health Rehabilitation Hospital. This past May, my father suddenly became very ill and required hospitalization followed by inpatient rehabilitation. We chose Encompass because of its excellent patient outcomes, and while we had hoped he would return home, new cognitive changes meant he needed a different long-term plan. During one of the hardest seasons of our family's life, the entire team at Encompass was compassionate, knowledgeable, and incredibly supportive. The physicians, therapists, and staff were always accessible and made sure we had the resources and guidance we needed. One moment I'll never forget was when a therapist named Kelly, who wasn't even assigned to my dad, saw how overwhelmed I was after a long day caring for my colicky newborn, touring assisted living facilities, and visiting the hospital. She gave me a snack from her own lunchbox and found me a quiet place to take an important phone call. That simple act of kindness brought me to tears. Our case manager, Lisa, was truly exceptional. She was always available to answer questions, provided invaluable resources and emotional support, and even checked on us after my dad's discharge. She made an incredibly difficult time feel manageable and helped our family create a plan we felt confident about. The outstanding care at Encompass extends far beyond rehabilitation. Their staff genuinely cares about both their patients and their families, and for that, we will always be grateful. I highly recommend Encompass Health Rehabilitation Hospital.

My recovery here has been stellar. The entire staff is amazing from Tanya therapy helped me achieve my Independence. Zhana the case manager made sure I had therapies set up to come to my house to continue my therapies. Juan the food service manager was so engaging to make sure the food was great. The nursing staff was stellar taking care of my every need and helped with my care day and night. The staff is engaging and friendly. What a great place to stay. The therapy techs were very engaging as well going past their requirements to make sure we were happy and engaged.

Still currently a patient here who suffered from a stroke, and I wanted to share my experience. The staff is very curtious and attentive. They go the extra mile. The hospital is clean. They come day or night. I never have to wait a long period of time for my calls to be answered. The therapy treatment is excellent.Mrs. Esperanza over OT makes sure I get cleaned up and dressed every day and that I have everything I need I appreciate her,she's a real sweetheart and Ms Taylor an OT that has so much compassion for people she will go that extra mile for you. Just because she cares,I love her.Mrs Devin my speech therapist has brought me a long way she's not just my therapist she's my friend.They really work me out in therapy. After 20 days here they still treat me like it's my first day. I have made so much progress. There is a PT therapist named James that is the absolute best therapist that anyone would want on their side. He works with you and for you and he won't let you give up or have a day where you don't want to do it. He will come to you and push you and I appreciate that. I'm as good as I am because he he wouldn't give up on me. Here you actually see your doctor everyday. They personally check on you to see your progress everyday and I really like that. I would really recommend Encompass to any of my family and friends or anyone that is looking to get better. And the food is good too!

My mother was a patient there for approximately two weeks. During her stay, I was unable to reach anyone via phone. When someone did answer I was transferred to a nurses station where no one ever answered. I was able to leave messages for the case worker a few times. My mother is incontinent and wears briefs full time but also CAN use the toilet. She was not taken to the toilet other than the one time I was there to tell them that she is able with assistance. She developed dermatitis in her genitals area as well as backside. She also had dermatitis under her breast folds. She was being seen by wound care prior to discharge and was supposed to have a referral for continued care at ALF. That referral was not processed. She arrived home to ALF and was back in the hospital for severe confusion. She had a severe UTI. There is no way the UTI developed in less than 48 hours of being discharged.
