Oak View Health and Rehabilitation Center

Some of the experiences were not too bad but for a size of oakview rehabilitation and not having good TV service for the residents and not having good food with another bad part about it. Some of the employees were nice and courteous and some are just downright ugly and mean. So that is why I'm only giving you a four I probably should give you worse than that but I'm not going to.

Do not send your loved ones here. My grandfather has called at least 4 times a week for basic things like blankets or a working bed since being placed here. On several occasions, the staff has had verbal and physical altercations with each other in the hallways in front of the residents and myself. I even heard the staff arguing with each other about how they purposely mistreat residents. Today my grandfather called stating he was in pain and being ignored. Given this was after their business hours, but upon arrival I spoke with a staff member and they warmly allowed me to come in a check on him. One other particular employee saw it fit to call the police claiming I stayed past visiting hours and refused to leave, ignoring the fact that I was let in by his fellow employee (after said visiting hours) and was not even there 5 minutes, actively leaving. Working with the VA to remove my grandfather from this awful place with below value staff and reporting several things seen with my own eyes to the state.

1. Smells like urine as soon as you walk through the door. 2. They do not come when patients press the call button. If your family/friend is not ambulatory they will not do well at all. 3. My mother came from the hospital with double pneumonia and it seems that they fraudulently charted her vitals until I told the nurses I was aware she did not receive any vital checks (to which they said they had checked her that day). My mother is alert and oriented, she was not lying. They started checking vitals after I asked about it. 4. There was a medication error by a nurse that was talking on their phone while administering medications. They call it a near miss. She gave my mom a pill container with 6-7 pills in the evening (she takes 2). My mom said it was not correct and the nurse yelled at her to take them. She said no and then the nurse took them back, realized it wasn’t the correct patient medication and proceeded to give her the correct one. They did not give her any evening medications for multiple days after that incident. 5. There are cockroaches, the window in her room was broken and there was tile falling off of the base of the wall. This place is not properly kept up. The only silver lining was that she did like PT and 1-2 of the staff that she had. She still has nightmares about being in this place. I will never send her back there and would not recommend it to anyone else.

This place needs to be shut down and called in. Some nurses are very nice. Most of them dont care for there patients and not wanting to do there job. Just wants to sit around and not clean there patients up. Make them lay in there mess. This is totally ridiculous. Not happy at all. Will not recommend to send my dogs or cats there either. Place is horrible. Place smells as soon as you walk in. Its needs to be turned in and shut down and reported.
