St. Johns County has one of the fastest-growing retiree populations in Florida, and a lot of adult children in Jacksonville, Nocatee, and Julington Creek are now arranging care for parents in St. Augustine, Ponte Vedra, World Golf Village, and the 55-plus communities off CR 210 and SR 16. The options run from a few hours a week of in-home companion help to assisted living, memory care, and skilled nursing. Florida licenses home health agencies and assisted living facilities through the Agency for Health Care Administration, and inspection records are public. Hurricane season matters here, since every licensed facility must have an evacuation and generator plan, and in-home caregivers need a backup if bridges to Vilano or Anastasia Island close. Distance is a practical issue too. A caregiver based in Hastings may not take a daily shift in Ponte Vedra.
Figure out the level of care honestly
Companion care covers meals, errands, and company. Personal care adds bathing, dressing, and transfers. Skilled care means a nurse. Assisted living is for people who need daily help but not a hospital. Be realistic about falls, memory, and medications, because choosing too little care now means moving again later.
Verify the license and read the inspections
In-home agencies and assisted living facilities in Florida are licensed by the Agency for Health Care Administration, and you can look up inspection reports and complaints online. For individual caregivers hired directly, ask for a Level 2 background screening. If anyone cannot show you a license number, walk away.
Ask about the hurricane plan
Every licensed facility in St. Johns County must have an emergency plan, and most now have generators for cooling. Ask where residents go if they evacuate and how families are notified. For in-home care, ask whether the agency staffs during a storm and what happens if your parent lives in a flood zone or across a bridge.
Visit at a meal and talk to residents
Tour an assisted living community at lunch, not at 10 a.m. when it is quiet. Watch how staff treat people who need help eating. Ask residents what they would change. Check the ratio of aides to residents on nights and weekends, since that is when problems happen.
Get every cost in writing
Assisted living base rates in St. Johns County rarely include the care itself. Levels of care, medication management, incontinence supplies, and transportation are often added on. Ask for the full fee schedule and the policy on rate increases. For in-home care, confirm the minimum hours per visit and the holiday rate.
