What should I do in a mental health emergency in St. Johns County?

Call or text 988, the Suicide and Crisis Lifeline, any time for yourself or someone else. If someone is in immediate danger, call 911. UF Health Flagler Hospital in St. Augustine has an emergency department, and the county is served by mobile crisis response teams that can be reached through 988 or local crisis lines. Under Florida's Baker Act, a person who is a danger to themselves or others can be evaluated involuntarily for up to 72 hours. Ask the office for its emergency protocol.

How do I find a child or teen therapist near Nocatee or Julington Creek?

Look for a therapist who lists child and adolescent work as a primary focus and ask what age range they see most. Play therapy credentials matter for younger children. St. Johns County schools have counselors and can refer families to community providers. Waitlists for child therapists in the northern county are often several weeks, so contact more than one practice. Ask whether parents are included in sessions and how the therapist communicates progress.

How to choose a therapist & counselor in Fernandina Beach & Amelia Island

Know the license types

In Florida, LMHC (mental health counselor), LCSW (clinical social worker), and LMFT (marriage and family therapist) are all licensed to provide therapy. Psychologists hold a doctorate and can do testing. Psychiatrists are medical doctors who prescribe. Registered interns work under supervision and often charge less. Verify any license on the Florida Department of Health site.

Match the therapist to the problem

Ask what the therapist actually treats most. Trauma, couples work, child and adolescent therapy, eating disorders, substance use, and OCD each call for specific training. A good therapist will tell you if your issue is outside their lane and refer you elsewhere.

Sort out the money before the first session

Many St. Johns County therapists are private pay only, typically $120 to $200 a session. Ask whether they take your insurance, offer a sliding scale, or provide a superbill so you can file for out-of-network benefits. Ask about cancellation fees too.

Decide on in-person versus video

Telehealth opens up therapists anywhere in Florida and avoids traffic on US 1 and CR 210. In-person tends to work better for young children and for some people with severe anxiety or trauma. Many local practices offer both, so you can start one way and switch.

Give it two or three sessions, then be honest

Fit matters more than credentials. If you do not feel heard after a few sessions, it is fine to say so and try someone else. Any good therapist would rather you find the right match than keep coming out of politeness.

How much does therapy cost in St. Augustine or Ponte Vedra without insurance?

A 50-minute individual session with a licensed therapist in St. Johns County typically costs $120 to $200 private pay in 2026, with psychologists and highly specialized providers charging $175 to $275. Couples and family sessions often run $150 to $250. Registered interns working under supervision commonly charge $50 to $100. Some practices offer a sliding scale based on income. Ask about fees and cancellation policies before you book the first appointment.

Why don't more therapists in St. Johns County take insurance?

Insurance reimbursement rates for therapy are often low relative to private pay rates, and the paperwork and authorization burden is heavy for a solo practice, so many therapists in Ponte Vedra, Nocatee, and St. Augustine opt out. Many will give you a superbill, which is an itemized receipt you submit to your insurer for out-of-network reimbursement. Check your plan's out-of-network mental health benefits; some PPO plans reimburse 50 to 70 percent after a deductible.

Can I see a therapist online if I live in St. Johns County?

Yes. Florida allows licensed therapists to provide telehealth to clients located in Florida, and most local practices now offer video sessions. You can also see a therapist based anywhere in the state, which helps when local waitlists are long. The therapist must hold a Florida license or be registered as an out-of-state telehealth provider with the Florida Department of Health. Insurance generally covers telehealth therapy the same as in-person visits.

Where can I get low-cost or free counseling in St. Augustine?

Options include community mental health providers that serve St. Johns County on a sliding scale, counseling centers that use supervised graduate interns at reduced rates, support groups through local churches and nonprofits, and services for veterans through the VA. The St. Johns County Health Department and local federally qualified health centers can help with referrals. Flagler College and University of North Florida counseling programs sometimes operate training clinics. Call 211 for a current list.
