SMA Outpatient - St. Augustine

Just called up this place for medical records because I use to do individual in person therapy sessions at this location they told me to check in at the main medical records center to fill out a release form I did all that and tried calling back and it seems like they fully blocked my number how crazy is that and also claiming on my files that I never was there or at any other place doing any treatment!!?

Don’t put your loved one here. If you care for someone, please do not bring them here. The employees here have no concern for ethics or human rights; they bring more harm than help; professionalism here it is a myth. It's as if respect and integrity doesn't exist in this facility. People are treated really poorly here. It’s traumatizing!!

The front desk ladies are kind and very helpful however I waited from 8:30 till 11:30am to get told I have to wait to hear back from someone else to reschedule an appointment...why? because the screener was ready to go to lunch. I appreciate that he atleast met with me to discuss a few things but I felt under valued when he recommended I reschedule with another office..I still haven't received a call back from anyone.

Front desk people are great and helpful. However, if you need a nurse remember to ask for Ms. Beth or you will have to call a supervisor for help. Jennifer will not answer the phone or respond to voicemails. Being rude doesn't make it easy for the clients who need help.
