Date of Incident: April 15, 2026 Time: 1:40 PM Location: UF Health Family Medicine – Callahan 450077 SR 200, Suite 12 Callahan, FL 32011-8109 To: UF Health Family Medicine Supervisor & Customer/Patient Relations Margo Walton, APRN UF Health Family Medicine – Callahan Copies Furnished To: By Certified Return Receipt Mail ● Agency for Health Care Administration (AHCA) ● Florida Department of Health (DOH) ● Blue Cross Blue Shield Member Services ● UF Health Corporate Compliance Office Re: Immediate Action Required – Denial of Care, Patient Abandonment, and Disability Discrimination Against a Long‐Term Patient My name is Joseph M. Jackson, and I am filing this formal complaint regarding the conduct of your clinic and the actions taken under your authority on April 15, 2026, at approximately 1:40 PM. I have been a patient of UF Health Callahan for over five years, insured through Blue Cross Blue Shield, and I rely on your office for continuity of care for multiple chronic medical conditions. My husband, Michael Schull, is recognized by the Department of Veterans Affairs to manage my medical needs, transportation, communication, and daily functioning. Michael is also an 80% service‐connected disabled Army Veteran, and he accompanies me to all appointments to ensure my safety. What Happened At check‐in, your staff demanded a $30 past‐due copay and a $60 copay for the day’s visit. Michael — acting in his official capacity as my VA‐appointed caregiver — explained clearly that we could pay the full amount the following day. Instead of acting with professionalism or basic human decency, your staff: ● Became hostile, rude, and confrontational ● Refused to allow me to be seen for my medically necessary appointment ● Ignored my caregiver’s attempt to resolve the situation ● Escalated the situation unnecessarily ● Ordered us to leave the clinic ● Publicly humiliated both a disabled veteran and his VA‐recognized caregiver This behavior is unacceptable under any circumstances — and indefensible when directed at a disabled patient and a disabled veteran caregiver. LEGAL AND ETHICAL VIOLATIONS 1. Patient Abandonment UF Health has treated me for over five years. Refusing medically necessary care without notice, transition, or justification meets the definition of patient abandonment under Florida law and national medical standards. 2. Violation of Florida’s Patient Bill of Rights (F.S. §381.026) Your staff violated multiple statutory rights, including: ● The right to timely access to care ● The right to respectful, dignified treatment ● The right to be free from humiliation, abuse, or harassment Your staff’s conduct was the opposite of these obligations. 3. Disability Discrimination (ADA Title III) I have documented disabilities. My caregiver is VA‐appointed. Denying care over a minor copay — especially when payment was offered the next day — constitutes discriminatory denial of services and failure to provide reasonable accommodation. 4. Potential Breach of Contract with Blue Cross Blue Shield As an in‐network provider, UF Health is contractually obligated to: ● Provide medically necessary care ● Follow BCBS copay collection policies ● Not deny care for minor outstanding balances Your staff’s actions appear to violate these obligations.
Ms. Margo is absolutely the best. She always takes her time and always listens. My husband and I just love her.
