In 2021 i had an injection that Dr Justin David Mann did the has left me with Radiculopathy it’s a long name for nerve damage. At the time he was going by David Mann. He tried to inject pain blockers directly into the main nerve. On the table I went into convulsions, the pain went the shock of pain went away from my head to my feel several times in seconds. The pain was so great that I almost fell off the table. This doctor said be still so I can get this the rest of this medicine in. At the time before all this occurred I was face down on the table listening to them plan their smoothie lunch. I reported this incident to one of his higher ups. The assistant called and apologized even the supervisor apologized, but I had no words from him. He was an arrogant doctor and he did it on purpose. Because I heard him say watch this before it happened. After I reported him I was given papers that I didn’t get before it happened to sign saying that I was told about side effects. I was working at the time, and the reason I was getting injections, so I can continue to work and sit without pain in my lower back. He disabled me. The social security disability doctor took one look at me and approved my disability. He damaged me, after I got home from the incident I had after shocks. Pain going from my head to my feet. Here it is 2026 and I’m still suffering with what he did to me.

I'd give them Zero stars if I could. Dr. Alphonso prescribed medications that made me sick, caused severe vision changes, and actually worsened my pain levels. When I called the the office to report the adverse side effects and request effective pain management, the person on the phone laughed and hung up. I called again only to be hung up on again. Really? I'm recovering from the most invasive surgery on the human body and it's a joke to them! I called back and got a different person that scheduled an appointment 22 days out. Nevermind, See You Next Tuesday! Update: Doctor suggested Ketamine Infusions for pain relief. So, Three different pills and a weekly IV infusion when I only needed 1 pill 2 to 3 times a day which I've taken before and it worked to manage my pain. Not to mention he wanted full diagnostics completed before any treatment. So, I left the office in pain once again and will continue to suffer. Thanks so much for helping my post surgical recovery!

Update: I saw Dr. Alphonso and explained the failed epidural and I was prescribed a muscle relaxer and reduced the pain a bit. Also am going to go to another physical therapy that Dr. Alphonso referred me to. Hopefully this recovery will end sooner than later. If I could, I would give them zero stars. So I have a herniated disc in my neck and I was giving an epidural about over a week ago. I am in severe pain and it made my pain way worse and every time I call I’m trying to tell them that I’m in pain and they’re not trying to do anything about it so then when I tell them that I need medication, no one is giving me any call back because the medication that I’m using now isn’t working so I’m trying to get put on a stronger inflammatory but no one wants to give me a call back and I’m in more pain than I was before so now I don’t know what to do.

Worst care I have ever received out of probably 50 neuro / pain drs. Had spinal cord surgery at age of 22 and was completely disregarded and disrespected. They forgot to fill my meds numerous times and were the rudest medical ppl I’ve ever encountered. Avoid at all costs
